Form 3508EZ applies to borrowers who meet any one of these three criteria:‌

  • Applied for the PPP loan as self-employed, an independent contractor or a sole proprietor with no employees.

  • Did not reduce salary or wages for any employee by more than 25%, and did not reduce the number or hours of their employees (excepting laid-off employees who refused an offer to return).

  • Did not reduce salary or wages for any employee by more than 25% during the covered period and experienced reductions in business activity as a result of health directives related to COVID-19.

How to Bank

As your local community bank, the health and safety of our customers, employees, and community are our top priority.  While this pandemic is greatly affecting our daily lives, it will not affect the excellent level of service you receive from Chelsea State Bank.

We remain open for all business.  Our bank lobbies are open by appointment only. And our drive-up banking is available at our three locations offering our full line of banking services. Feel free to call us for any assistance you may need at 734.475.1355.

We urge you to use our online banking services and mobile app as much as possible.  These services are available to you 24/7, and you can perform most of your regular banking activities using these digital tools, including:

  • Online Banking – Check account balances, view and print statements, transfer money between your CSB accounts or transfer to outside accounts, transfer money to your friends and family, and pay bills, including utility, phone, and loan payments.
  • Mobile Banking – Deposit checks and manage most all your banking with our mobile app.  The $1 fee for business mobile deposit will be waived.
  • Apply Online -- Personal checking accounts and loans
  • Debit Cards – Use your debit card to get cash back at point of sale at some stores
  • Telephone Banking – Dial 734-411-myCSB or call 734-475-1355.  All telephone banking fees are waived until further notice.
  • Remote Deposit Capture – For businesses

  • Loan payment options, including interest-only payments and payment deferrals upon request
  • Provide lines of credit

April 27, 2020 UPDATE

Additional funding was granted to the PPP program and the program will resume at 10:30 a.m. on Monday, April 27.  An additional $310 billion was approved, including $60 billion set aside for smaller institutions.  As a result, we are now accepting applications. Please use the application links below. We will keep this message updated as we hear more on additional funding. Thank you.

The newly enacted federal Coronavirus Aid, Relief, and Economic Security (CARES) Act provides a variety of additional emergency relief programs for consumers and businesses, including the Paycheck Protection Program noted below. We are actively working to operationalize these programs as quickly as possible so that we can further assist our customers in maximizing their relief under the Act’s provisions. We will begin accepting applications for small business and sole proprietorships on Friday, April 3 and independent contractors and self-employed individuals on Friday, April 10. As we collectively navigate these uncertain times, the safety and well-being of our customers, employees and the communities we serve remains our top priority. We will continue to update our website as changes require.

SBA Paycheck Protection Program (PPP)

Chelsea State Bank is an authorized lender by the SBA to administer the Paycheck Protection Program (PPP). The PPP authorizes up to $349 billion in forgivable loans to small businesses, nonprofits, veterans organizations and tribal concerns to pay their employees during the COVID-19 crisis. Sole proprietors and self-employed individuals are eligible as well. Loan terms are the same for everyone. The loan amounts will be forgiven as long as:

  • The loan proceeds are used to cover payroll costs, mortgage interest, rent, and utility costs over the 8-week period after the loan is made
  • Employee and compensation levels are maintained
  • Laid off employees are brought back onto payroll

While we wait for the SBA to provide further details on the PPP, you can begin to gather information for your loan application. We believe that the following information will be required:

  • 2019 IRS Quarterly 940, 941 or 944 payroll tax reports
  • Payroll reports for 2019
    • Gross wages for each employee, including officer(s) if paid W-2 wages
    • Paid time off for each employee
    • Vacation pay for each employee
    • Family medical leave pay for each employee
    • State and local taxes assessed on an employee’s compensation
    • 1099s for independent contractors for 2019
  • Documentation showing total of all health insurance premiums paid by the company owner(s) under a group health plan.
    • Include all employees and the company owners
  • Document the sum of all retirement plan funding that was paid by the company owner(s) (do not include funding that came from employees out of their paycheck deferrals).
    • Include all employees and the company owners
    • Include 401K plans, Simple IRA, SEP IRAs

Sole proprietors and self-employed individuals, need to complete their own application with supporting payroll documentation for 2019.
